Capstone Final Reflection (2 pages maximum; single spaced; at least 750-800 words)

In this final report, reflect on your own development as a Health Sciences major in your final semester at 

Cal State East Bay. Given your near-completion of this class, your community partner experience, and 

your time as an undergraduate student your final assignment is a piece of scholarly reflection and 

analysis. 

Student Summary:

Concise summary (1 short paragraph) that includes your name, areas of academic concentration, what 

brought you to Cal State East Bay and how they have grown and changed as a result of their 

undergraduate experience and their plans after graduation from Cal State East Bay. Provide a summary 

of your personal strategic plan, including a statement of your values, vision, mission and strategies.

Reflections on the Community Partner Experience: 

In this section, introduce the organization/agency (community partner) and the specific project 

completed this semester. Reflect on your learning objectives and to the extent you were able to 

complete those objectives. What was your main contribution and what did you learn from the 

Rubric and assignment instructions is also uploaded 

community partner? In your narrative, include what this project has taught you about population health, 

and the specific health policy issues that are relevant to the project. How did this project connect to 

your academic learning and what might it lead to next? [You are essentially summarizing and reflection 

on the work you completed in your activity section this semester].

Reflections on Diversity: 

What are the key lessons you have taken away about diversity from your Health Sciences coursework at 

Cal State East Bay? How has your capacity to interact openly and respectfully with individuals changed, 

particularly marginalized individuals, across the full range of human diversity including, but not limited 

to race, ethnicity, religion, gender, sexual orientation, age, ability, socioeconomic status, health, and 

mental health? What lessons do you hope to bring with you about cultural self-awareness, knowledge 

and respect for diverse perspectives in the health field as you embark on your future career or 

graduate/professional study?

Reflections on Leadership and Team Work: 

Reflect on your leadership and team work experiences throughout the Health Sciences program. 

Specifically, include a reflection on your communication skills, accountability, conflict management and 

leadership skills.
