CLO3: Design a process safety management programme for
process industries.
PURPOSE:
To enable the learner to demonstrate understanding of the concept and
application of process safety management elements in preventing a catastrophic
incident in process industries.
REQUIREMENT:
Individual Assignment
On July 5th, 2018, approximately 6:15 pm, a fire strike out at Kemaman
Bitumen Company Sdn Bhd located at Teluk Kalong Industrial Area, Terengganu.
Four contract workers were injured due to the blaze with one suffered seriously
burnt and treated at Burns Unit, Hospital Sultanah Nur Zahirah in Batu Burok,
Kuala Terengganu. More than 100 firefighters were battling a raging fire that
destroyed three huge crude oil tanks. Of the three tanks, two have been gutted
and firefighters were trying to put out the fire at the third tank using foam
so that it does not spread to other tanks. It was reported that the victims
were carrying out hot work near the facility that had caused the fire.The blaze
was described as the biggest in Terengganu for years.

Discuss the violation to process safety
management elements; and
(b)
Explain how such incident can be prevented
and if the catastrophic episod occurs, how to mitigate and minimise the impact
to people, property and surrounding.
