For this assignment you
will be working in your same groups to develop survey items to measure the outcomes for the program you
designed (COPY OF THE OUTCOMES FOR THE PROGRAM DESIGNED IS ATTACHED, PLEASE USE
IT FOR THIS ASSINGMENT) in Module 1 AND 2
Using the program
design that you created in Module 1 and the outcome of your design in
Module 2, develop a set of questions for a survey to be administered to program
participants. The survey questions should measure program outcomes.
