- Participants do NOT sign this form, participants will instead sign the Informed Consent Form.
- Excluded research is defined by theIRB Handbook [DOWNLOAD] as, “Instructors assigning research as part of a class assignment take responsibility for protection of human subjects in the student research projects. The class instructor determines that the student research projects meet criteria for protection of human subjects and that the projects can be excluded from Institutional Review Board review. Projects in this category are confined to the specific class and end at the termination of the class” (IRB Handbook, 2012).
