Task: Select a child who has cerebral palsy (GMFCS level V) with associated multiple impairments and medical complications. Imagine that the child is moving abroad to, say, Sweden.
Write a comprehensive report for the parents to take with them giving a summary of the child’s
strengths and difficulties, information about what has happened in the past, and what you
think the child’s needs are now and in the near future.
You don’t know who will be responsible for the health follow up of the child so this should
inform a general practitioner or child development centre as well as being comprehensible to
the family.
The purpose of this assessment task is to demonstrate your understanding of the complexity
of the child’s problems and your ability to summarise this and convey the information to
others.
